Qualifications:
MD or DO degree
Active U.S. medical license
Experience in Family Medicine, Internal Medicine, Gerontology, Palliative Care, or Home Care
Strong background in adult primary care and chronic disease management
Interest or experience in value-based care preferred
Excellent communication and interpersonal skills
