- Family Nurse Practitioner
Description
The Nurse Practitioner (NP) provides comprehensive primary healthcare services to patients of Center Street Community Health Center in accordance with the scope of practice defined by the Ohio Board of Nursing, established written protocols, and individual clinical competencies. The NP works collaboratively with the Clinical Manager, physicians, and interdisciplinary care team to deliver high-quality, patient-centered care. Additional duties may be assigned in alignment with organizational policies, procedures, and funding requirements.
Essential Duties and Responsibilities
Serve as an integral member of the health care team, fostering collaborative and professional relationships with physicians and other clinical staff.
Perform comprehensive physical examinations and preventive health services, including immunizations, developmental screenings, and age- and gender-appropriate laboratory tests and procedures.
Diagnose and manage common, acute, and stable chronic health conditions; consult with or refer patients with complex or atypical conditions to appropriate providers or specialists.
Prescribe medications, treatments, and medical devices in accordance with Ohio state laws and organizational protocols, including services related to family planning and perinatal care.
Accurately document patient encounters in the electronic medical record (EMR), utilizing appropriate assessment tools and ensuring timely and complete charting.
Respond to medical emergencies and provide life-saving interventions and first aid as necessary.
Educate patients and families on diagnoses, treatment plans, and preventive care, ensuring understanding and supporting informed decision-making.
Deliver culturally competent, patient-centered care that considers social, psychological, environmental, and economic factors impacting health.
Coordinate care, including referrals and follow-up services, to ensure continuity and comprehensive treatment.
Provide counseling and ongoing management for patients with stable chronic conditions; refer patients with unstable conditions as appropriate.
Adhere to infection control standards, including universal precautions, in all clinical activities.
Utilize medical supplies and resources efficiently and cost-effectively.
Participate in outreach activities and community-based initiatives as needed.
Engage in Quality Assurance (QA) and Quality Improvement (QI) programs and organizational initiatives.
Maintain compliance with all applicable federal, state, and local regulations, as well as organizational policies.
Requirements
Qualifications
Current licensure as a Nurse Practitioner in the State of Ohio.
Certification by a recognized national certifying body (e.g., AANP, ANCC).
Current DEA registration (or ability to obtain).
Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) preferred.
Experience in a community health center, primary care, or underserved population setting preferred.
One (1) year working in a primary care outpatient environment.
Strong clinical, communication, and interpersonal skills.
