Rocky Mountain PACE (Program of All Inclusive Care for the Elderly) is seeking a dedicated Physician to join our growing team and help shape the future of participant-centered care.
About the Role
The PACE Physician works as a key member of our Interdisciplinary Team (IDT), providing high-quality, person-centered medical care to older adults who wish to remain independent in the community. This role has a unique emphasis on clinical education and support for our Nurse Practitioners, ensuring they are empowered, clinically aligned, and able to deliver exceptional care.
You will play a vital role in strengthening clinical practice, supporting professional growth, and helping standardize clinical pathways across our PACE program.
What You’ll Do
Provide primary medical care oversight and instruction to PACE participants within an interdisciplinary care model.
Serve as the clinical mentor and educator for Nurse Practitioners, offering real-time consultation, case review, and structured development support.
Guide clinical decision-making, reinforce evidence-based practices, and participate in the creation and refinement of clinical protocols.
Collaborate closely with the Medical Director to promote consistent, high-quality medical practice throughout the program.
Participate in IDT care plannings.
Support regulatory readiness and quality improvement initiatives.

Requirements
What We’re Looking For
Board-certified or board-eligible physician (Internal Medicine, Family Medicine, or Geriatrics preferred).
Experience working with older adults, complex care, or PACE strongly preferred.
Demonstrated interest or experience in clinical teaching, coaching, or mentoring advanced practice clinicians.
Strong communication skills and a collaborative mindset.
Passion for person-centered, community-based care.
