We are an established concierge primary care practice seeking a compassionate, motivated Nurse Practitioner to join our growing team.

Our practice specializes in providing personalized medical care to patients in the comfort of their homes, primarily serving the geriatric population throughout North San Luis Obispo County.

This is an excellent opportunity for an NP who enjoys building meaningful patient relationships, practicing personalized medicine, and working in a supportive, physician-led environment.

Key Requirements

Responsibilities:
Provide primary care to approximately 6–8 patients per workday through scheduled home visits.
Conduct comprehensive patient assessments, diagnose and manage acute and chronic conditions, and develop individualized treatment plans.
Provide occasional telemedicine consultations.
Maintain accurate, timely electronic medical records.
Collaborate with the supervising/supporting physician and other healthcare professionals to coordinate patient care.
Participate in required chart reviews and quarterly performance assessments.
Qualifications:
Current California Nurse Practitioner license and appropriate certifications.
Strong clinical assessment and communication skills.
Ability to work independently while collaborating with a physician.
Compassionate approach to caring for older adults.
Previous experience in primary care, geriatrics, or home health is preferred but not required.
