Nurse Practitioner (Multiple Openings – Choose Your Service Area)
Location: Oregon (Metro or Central Travel Regions)
Compensation: $140 per visit + mileage reimbursement

About the Role:
We are hiring multiple Nurse Practitioners to support our growing Oregon markets. This is a flexible, field-based role focused on conducting Annual Wellness Visits (AWVs) in patients’ homes.
No prescribing. No medication management. No clinic work.
Just meaningful patient interactions and assessments.
Choose Your Service Area
We are currently hiring for the following traveler/regional roles:
- Metro Oregon Traveler - Covers: Washington & Clackamas Counties
- Central Oregon Traveler - Covers: Marion, Yamhill & Columbia Counties
- Key areas include Salem, McMinnville
You can choose the service area that best fits your location and travel preference.
What You’ll Do
- Complete 6–9 in-home health assessments per day
- Review medical history and identify risk factors
- Collect vitals, labs, and screening data
- Document findings in EMR
- Educate members on recommended next steps
- Coordinate follow-up care when red flags are identified
- Travel within your selected region (Metro OR: Washington & Clackamas Counties OR Central OR: Marion, Yamhill & Columbia Counties)

Requirements
- Must have license/certification: APRN, AANP, AANC
- Must be registered with Medicare and/or Medicaid
- Valid proof of vaccination
- CAQH number
- Valid BLS or ACLS
- Valid driver’s license + reliable car
- Able to work independently in the field
