- Nurse Practitioner - Endocrinology
Description
AMCR is seeking a dedicated and compassionate Nurse Practitioner to join our Endocrinology team, focusing primarily on the care of patients with diabetes, obesity along with other metabolic disorders. The ideal candidate will possess advanced clinical skills and a passion for improving the health outcomes of individuals with diabetes through comprehensive education, management, and support.
Key Responsibilities:
Patient Assessment and Management:
Conduct thorough patient assessments, including health histories and physical examinations.
Develop and implement individualized care plans for diabetes management.
Utilize a variety of data sources, including CGMs, pumps, EMR, SDoH, etc. to track patient progress and adjust care plans as needed.
Prescribe and manage medications, including insulin and other diabetes-related therapies.
Monitor and evaluate the effectiveness of treatment plans, making adjustments as needed.
Ability to absorb already robust patient panel and serve a broad PCP referral network.
Patient Education:
Provide education to patients and families about diabetes management, self-monitoring, nutrition, and lifestyle modifications.
Assist patients in starting and persisting with Continuous Glucose Monitors (CGMs) and Insulin Pumps
Offer resources and support for diabetes self-management and promote adherence to treatment plans.
Virtual Care
Experience coordinating patient care with virtual care providers for Chronic Care Management, Remote Patient Monitoring, Diabetes Education, etc.
Experience delivering care virtually a plus
Care Team Collaboration:
Serve as an integral part of the clinical Care Team supporting a broad range of patients with diabetes and other metabolic conditions
Collaborate with endocrinologists, dietitians, and other healthcare professionals to ensure a multidisciplinary approach to patient care.
Participate in Care Team case discussions and care coordination for complex patients.
Clinical Procedures:
Perform necessary clinical procedures related to diabetes management and other metabolic conditions (e.g. Ultrasound, DEXA, Body Composition, FibroScan, ABI, Thyroid Thermal Ablation for Thyroid Nodules)
Documentation:
Maintain accurate and thorough patient records, ensuring compliance with regulatory and billing requirements.
Document patient progress and outcomes in electronic health records.
Quality Improvement:
Participate in quality improvement initiatives to enhance patient care and clinic operations.
Stay current with advancements in diabetes care and participate in continuing education opportunities.
Requirements
Qualifications:
At least 6-12 months experience caring for patients with diabetes in an outpatient endocrinology, primary care or family care practice setting
A strong desire to improve care for people with diabetes and healthcare in general
Current state licensure as a Nurse Practitioner with prescriptive authority.
Strong communication and interpersonal skills, with the ability to build rapport with patients and families.
Exceptional organizational skills and attention to detail.
